Sycamores’ CS: Housing Supports assists members in finding viable housing and then helps ensure members have the support and means to maintain housing.
These services are offered at NO COST to you as part of your Medi-Cal health plan.
Our ECM team works with members who have complicated needs and challenges that make it difficult to improve overall health. This could include health conditions or challenges such as not having a place to live.
At Sycamores, we understand the importance of individual care specific to your situation, so you have what you need to be healthier.
A Care Manager listens to your needs and connects you to nearby resources.
How do I know if I qualify for this program?
You are a Medi-Cal member with a Managed Care Plan
Anthem, Health Net, Kaiser, L.A. Care or Molina
You live in Los Angeles County
You would like support in addressing any of the following:
Experiencing homelessness
Having had 2 or more hospitalizations
Needing to work with a therapist or counselor to manage mental health or substance use
Moving out from a hospital or a long term care facility
Are pregnant or currently postpartum

The Medi-Cal services you are currently getting now will NOT be taken away. You can still see your same doctors, but now you can get extra help.
Sycamores’ CS program can help you find and secure stable housing.
Our CS team works with members who need assistance with locating and maintaining housing by helping members find a potential home, securing housing deposits, providing rental education, advocacy and more.

Guide you through the steps involved with finding housing
Help you complete the application
Connect you with benefits and other resources that can lower your rent costs and cover housing-related expenses
Housing Deposits
We are here to connect you with resources or financial assistance to make sure your move-in goes well.
This could mean helping you cover some housing expenses related to moving
Ongoing success
Once you have found housing, our CS team can help you:
Master the basics of renting
Build good landlord relations
Coordinate with others who are supporting your success
